Easterseals Iowa Case Management promotes independence by proactively partnering with individuals, their families, and support network. Our focus is not only on coordinating care, but on collaborating to create individualized support plans that reflect each individual's unique circumstances. We act as knowledgeable connectors, identifying and linking to the most appropriate services, community resources, and support systems —whether relating to healthcare, housing, education, employment, or social inclusion. Our team is knowledgeable, proactive and supports individuals in being an active role in shaping their own outcomes.
